Agreement and Release Form:
Once you have been accepted, go to the online portal and read and agree to the terms of the general agreement and release form. Students who have not completed this step will not be permitted to participate in a program.
A copy of the Agreement and Release form is available FOR READING ONLY here as a PDF document, but DO NOT PRINT this version. It will only be accepted by CFIS from students under age 18, who must obtain the signature of a parent or guardian.
